New Screening Tool for Migraine Headaches

Researchers have developed a new tool that can accurately identify patients with chronic migraine (CM)

The screening questionnaire — called the ID-CM — can potentially be used by physicians or by patients themselves.

The tool “… may be helpful for people living with chronic migraine who don’t realize there’s a term for what they’re living with,” she said. “It might help connect them with specialist health care professionals who can treat them well, and it may help connect them with the right treatment.”

Among other things, the final 12-item screening tool asks respondents how often their pain is moderate or severe, how often they’re unusually sensitive to light and sound, how often they feel nauseated or sick to their stomach, and how often they worry about missing work, school, or social events because of headaches.

Despite its substantial economic and quality-of-life burden, chronic migraine remains underdiagnosed and undertreated.

New Scoliosis Classification System

A classification system was developed by a group of experienced surgeons to identify early-onset scoliosis. The system will serve as a guide for new research and clinical communication when treating the disorder.

Fifteen surgeons participated in a nominal group technique with the goal of developing a consensus-based classification system for early-onset scoliosis. During the course of three group meetings and two rounds of surveying, the surgeons generated a list of factors important to managing early-onset scoliosis using information gathered from a standard literature review, semi-structured interviews and open-forum discussion.

Three core variables — major curve angle, etiology and kyphosis — were identified using the nominal group technique. Thirty patients with early-onset scoliosis were then analyzed using the system for reliability testing.

According to the researchers, the mean kappa value for etiology was substantial at 0.64, whereas the mean kappa values for major curve angle and kyphosis were in nearly perfect agreement, at 0.95 and 0.93, respectively.

The surgeons’ final classification system consisted of continuous age prefix, etiology, major curve angle, kyphosis variables and an optional progression modifier.

Annular Tears and Accelerated Disc Degeneration

It is questionable whether an annular tear (AT) is a predictor for accelerated degeneration of the intervertebral discs. The aim of the present study published in the European Spine Journal, was to answer this question via a matched case–control study design that reliably eliminates potential confounders.

Presence or absence of annular tear, defined as a hyperintense lesion within the annular fibrosus on T2-weighted non-contrast MRI images, was documented in 450 intervertebral lumbar discs of 90 patients who could be followed up for at least 4 years with MRI.

The study concluded that Discs with a Pfirrmann grade >2 with an annular tear, defined by a hyperintense signal intensity on MRI, are not prone to accelerated degeneration if compared to discs without an annular tear. Therefore, the presence of an annular tear per se does not predict accelerated disc degeneration.

Impact of Chiropractic Services at an On-Site Health Center

A recent study in the Journal of Occupational Environmental Medicine compared the influence of employer-sponsored, on-site chiropractic care against community-obtained care on health care utilization.

This was a retrospective claims analysis of members of a single employee health plan receiving chiropractic care on-site or off-site from 2010 to 2012.

Utilization differences were evaluated by having 1 health care event or more, including radiology or clinical visits.

The study concluded that compared with off-site care, on-site chiropractic services are associated with lower health care utilization. These results support the value of chiropractic services offered at on-site health centers.

Exercise and Gut Microbiome

The commensal microbiota, host immunity and metabolism participate in a signaling network, with diet influencing each component of this triad.

In addition to diet, many elements of a modern lifestyle influence the gut microbiota but the degree to which exercise affects this population is unclear. Therefore, the authors of a recent study published in the journal GUT, explored exercise and diet for their impact on the gut microbiota.

As expected, athletes and controls differed significantly with respect to plasma creatine kinase (a marker of extreme exercise), and inflammatory and metabolic markers. More importantly, athletes had a higher diversity of gut micro-organisms, representing 22 distinct phyla, which in turn positively correlated with protein consumption and creatine kinase.

The results of the study provide evidence for a beneficial impact of exercise on gut microbiota diversity but also indicate that the relationship is complex and is related to accompanying dietary extremes.

Artificial Sweeteners and Insulin Resistance

Nonnutritive sweeteners (NNS), such as sucralose, have been reported to have metabolic effects in animal models. However, the relevance of these findings to human subjects is not clear. The authors of a recent study evaluated the acute effects of sucralose ingestion on the metabolic response to an oral glucose load in obese subjects.

Compared with the control condition, sucralose ingestion caused a greater incremental increase in peak plasma glucose concentrations; a greater incremental increase in insulin area under the curve; a greater peak insulin secretion rate decrease in insulin clearance; amongst other findings.

There were no significant differences between conditions in active glucagon-like peptide 1, glucose-dependent insulinotropic polypeptide, glucagon incremental AUC, or indices of the sensitivity of the β-cell response to glucose.

The study concluded that these data demonstrate that sucralose affects the glycemic and insulin responses to an oral glucose load in obese people who do not normally consume NNS.

Security of Mobile Data

Federal Trade Commissioner Julie Brill, speaking in Washington back in July, expressed concern about the way apps on smartphones and mobile devices are siphoning sensitive health data, and how some of that information may then be shared with third parties.

After a panel discussion hosted by U.S. political site The Hill, Brill told Reuters that many companies now prefer to focus on how data is used, because that is where “the rubber hits the road when it comes to patient harm.” Developers should give consumers more tools and “robust choice mechanisms” before any sensitive data is collected and stored.

“We don’t know where that information ultimately goes,” Brill told the panel. “It makes consumers uncomfortable.” She has put pressure on Congress to pass laws prohibiting the collection of personal information under false pretenses.

The debate around the gathering of consumer data is intensifying as Silicon Valley tech companies take a more active interest in mobile health. Apple Inc. and Google Inc. revealed new health-focused services for apps developers in recent months, dubbed Google Fit and HealthKit.

Brill’s comments followed a May report in which the FTC revealed the results of a study of mobile health-app developers, which found that a good portion collect consumer health data and give it to third-party entities.

Road to ICD-10

The Centers for Medicare & Medicaid Services (CMS) has released a new webcast introducing the “Road to 10” tool.

Accessible through the “Road to 10” link on the CMS website, the webcast covers the history of the International Classification of Diseases (ICD) and the benefits of ICD-10. This is the first in the new “Road to 10” webcast series.

Five more webcasts will follow—all aimed at helping small practices get ready for ICD-10 by the October 1, 2015, compliance date.
