Subjective Well-Being, Health and Aging

Subjective wellbeing and health are closely linked to age.

Three aspects of subjective wellbeing can be distinguished—evaluative wellbeing (or life satisfaction), hedonic wellbeing (feelings of happiness, sadness, anger, stress, and pain), and eudemonic wellbeing (sense of purpose and meaning in life).

Authors of a recent The Lancet journal study reviewed recent advances in the specialty of psychological wellbeing, and present new analyses about the pattern of wellbeing across ages and the association between wellbeing and survival at older ages.

The review found that older people with illnesses such as coronary heart disease, arthritis, and chronic lung disease show both increased levels of depressed mood and impaired hedonic and eudemonic wellbeing.

Wellbeing might also have a protective role in health maintenance.

The authors identify that eudemonic wellbeing is associated with increased survival; 29·3% of people in the lowest wellbeing quartile died during the average follow-up period of 8·5 years compared with 9·3% of those in the highest quartile.

The authors conclude that the wellbeing of elderly people is an important objective for both economic and health policy. Present psychological and economic theories do not adequately account for the variations in patterns of wellbeing with age across different parts of the world.

The apparent association between wellbeing and survival is consistent with a protective role of high wellbeing, but alternative explanations cannot be ruled out at this stage.

Texting and the Spine

Sixty pounds is roughly the weight of four adult-sized bowling balls. Or six plastic grocery bags worth of food. Or an 8-year-old.

It is also, according to a new calculation published in the journal Surgical Technology International, the amount of force exerted on the head of an adult human who is looking down at her phone.

Kenneth Hansraj, a New York back surgeon, found this figure using a computer model of a human spine. An average human head weighs about 10 to 12 pounds, and tilting it down to check Facebook, send a text, or to Google the weight of an a human head increases the gravitational pull on said cranium.

“As the head tilts forward the forces seen by the neck surges to 27 pounds at 15 degrees, 40 pounds at 30 degrees, 49 pounds at 45 degrees and 60 pounds at 60 degrees,” Hansraj writes in the paper.

According to Nielsen, Americans spend about an hour on their smartphones each day. Unless you train yourself to stare straight ahead into your iPhone screen, you could be continually stressing your spine. “These stresses,” Hansraj writes, “may lead to early wear, tear, degeneration, and possibly surgeries.”

Soda and Cell Aging

A study, recently published in the American Journal of Public Health, tested whether leukocyte telomere length maintenance, which underlies healthy cellular aging, provides a link between sugar-sweetened beverage (SSB) consumption and the risk of cardiometabolic disease.

The authors of the study examined cross-sectional associations between the consumption of SSBs, diet soda, and fruit juice and telomere length in a nationally representative sample of healthy adults.

The study population included 5309 US adults, aged 20 to 65 years, with no history of diabetes or cardiovascular disease, from the 1999 to 2002 National Health and Nutrition Examination Surveys. Leukocyte telomere length was assayed from DNA specimens.

Diet was assessed using 24-hour dietary recalls. Associations were examined using multivariate linear regression for the outcome of log-transformed telomere length.

Results of the study found that adjustment for sociodemographic and health-related characteristics, sugar-sweetened soda consumption was associated with shorter telomeres. Consumption of 100% fruit juice was marginally associated with longer telomeres. No significant associations were observed between consumption of diet sodas or noncarbonated SSBs and telomere length.

Artificial Sweeteners and Glucose Intolerance

In mice, artificial sweeteners lead to alterations in the microbiome and changes in glucose absorption.

Noncaloric artificial sweeteners, such as saccharine, sucralose, and aspartame, were introduced to control body weight and lower risk for diseases linked to obesity.

Yet the epidemic of type 2 diabetes and obesity has coincided with introduction of noncaloric sweeteners. Most of us assume that the epidemic of obesity and type 2 diabetes led to more use of noncaloric sweeteners.

However, researchers in Israel report that the opposite might be true: Use of noncaloric sweeteners might have contributed to the epidemic.

Mice that are given noncaloric sweeteners develop glucose intolerance quickly, compared with mice that are given sucrose or glucose. Introduction of dietary noncaloric sweeteners promptly alters the mouse microbiome to favor biochemical pathways that enhance absorption of calorie-rich glucose and short-chain fatty acids.

Exercise and Chronic Pain

The purpose of a 2001 review published in The Clinical Journal of Pain, was to determine how effective exercise is in the treatment of chronic pain.

The literature search identified three systematic reviews and three randomized controlled trials addressing the effectiveness of exercise for the management of chronic low back pain, one systematic review and one randomized controlled trial addressing chronic neck pain, two systematic reviews and three randomized controlled trials addressing upper extremity pain, and three randomized controlled trials addressing fibromyalgia.

The study concluded that exercise is effective for the management of chronic low back pain for up to 1 year after treatment and for fibromyalgia syndrome for up to 6 months (level 2).

There is conflicting evidence (level 4b) about which exercise program is effective for chronic low back pain. For chronic neck pain and for chronic soft tissue shoulder disorders and chronic lateral epicondylitis, evidence of effectiveness of exercise is limited (level 3).

Biopsychosocial approaches to the treatment of chronic pain.

Biopsychosocial treatments address the range of physical, psychological, and social components of chronic pain.

A review published in The Clinical Journal of Pain, sought to determine how effective unimodal and multimodal biopsychosocial approaches are in the treatment of chronic pain.

The literature search identified three systematic reviews of the literature and 21 randomized controlled trials to provide the evidence for this review.

The study concluded that multimodal biopsychosocial treatments that include cognitive-behavioral and/or behavioral components are effective for chronic low back pain and other musculoskeletal pain for up to 12 months (level 2).

There is limited evidence (level 3) that electromyogram feedback is effective for chronic low back pain for up to 3 months. The remaining evidence of longer-term effectiveness and of effectiveness of other interventions was inadequate (level 4a) or contradictory (level 4b).

Future studies of cognitive-behavioral treatments should be condition specific, rather than include patients with different pain conditions.
