Age and Lumbar Fusion
The aim of a study published in the journal Spine, was to assess the patient-rated outcome and complication rates associated with lumbar fusion procedures in three different age groups.
There is a general reluctance to consider spinal fusion procedures in elderly patients due to the increased likelihood of complications.
A total of 707 consecutive patients were included in the study. The preoperative comorbidity status differed significantly between the age groups, with the highest scores in the geriatric group.
Medical complications during surgery were lower in the younger age group than in the older and geriatric groups; surgical complications tended to be higher in the elderly group.
The study concluded that Despite greater comorbidity and complication rates in geriatric patients, the patient-rated outcome was as good in the elderly as it was in younger age groups up to 2 years after surgery.
These data indicate that geriatric age needs careful consideration of associated risks but is not per se a contraindication for fusion for lumbar degenerative disease.

Mindfulness Based Stress Reduction and LBP
A study was published in JAMA to evaluate the effectiveness for chronic low back pain of MBSR vs cognitive behavioral therapy (CBT) or usual care.
Mindfulness-based stress reduction (MBSR) has not been rigorously evaluated for young and middle-aged adults with chronic low back pain.
Interventions included BT (training to change pain-related thoughts and behaviors) and MBSR (training in mindfulness meditation and yoga) were delivered in 8 weekly 2-hour groups. Usual care included whatever care participants received.
The study concluded that among adults with chronic low back pain, treatment with MBSR or CBT, compared with usual care, resulted in greater improvement in back pain and functional limitations at 26 weeks, with no significant differences in outcomes between MBSR and CBT.
These findings suggest that MBSR may be an effective treatment option for patients with chronic low back pain.
