Nonsurgical Treatment Choices by Individuals with Lumbar Intervertebral Disc Herniation
The objectives of a study published in the American Journal of Physical Medicine and Rehabilitation, were to:
- Evaluate differences between patients with lumbar intervertebral disc herniation who received physical therapy (PT) and those who did not;
- Identify factors associated with receiving PT; and
- Examine the influence of PT on clinical outcomes over the course of 1 year.
This study was based upon an observational cohort study using data from the Spine Patient Outcomes Research Trial was conducted.
This study included 363 patients with intervertebral disc herniation who received nonsurgical management within 6 weeks of enrollment.
The study concluded that many patients with intervertebral disc herniation seek secondary care for persisting symptoms and pursue nonsurgical management.
The best management strategy is unclear and further research is needed to examine appropriate sequencing and selection of treatment.

Ankle Manipulation and Functional Performance
A study was published in the Journal of Bodywork and Movement Therapies to evaluate the immediate effect of talocrural joint manipulation (TCJM) on functional performance of athletes with chronic ankle instability (CAI).
Forty athletes (18 males, 22 females) with chronic ankle instability divided into talocrural joint manipulation group and sham manipulation group.
Talocrural joint manipulation was performed as a quick thrust on the involved talus, in the posterior direction. Sham manipulation was maintaining the same position, without any thrust.
Functional performance of athletes was assessed with single leg hop; speed and Y balance tests, before and after the interventions.
The study concluded that talocrural joint manipulation can significantly increase the functional performance of athletes with chronic ankle instability and can be an effective supplementary treatment for these subjects.
However, this was a pre-post study and future studies with long-term follow-ups may provide more reliable results about the long-term effectiveness of this type of treatment.
