Smoking and Arthritis
Smoking was a strong predictor of more severe disease in patients with axial spondyloarthritis (AxSpA), with people who had ever smoked having worse disease activity and worse quality of life than those who had never smoked, researchers reported.
In addition, current smokers fared worse than those who had quit, according to researchers from the University of Liverpool.
People who had ever smoked had evidence of worse disease across the measures, after adjusting for age, sex, BMI, and HLA-B27 status, Zhao reported. For example, on the BASDAI, ever smokers had 1 unit score worse disease activity compared to people who had never smoked.
Furthermore, people who had quit smoking had better disease activity than those who continued to smoke.
These study findings provide yet another reason to encourage patients with AxSpA to quit smoking, and to do so before they accumulate a large number of pack-years.
In a related story, it was found that around 1 in 5 people with axial spondyloarthritis meet the criteria for fibromyalgia, and this group fared worse on multiple measures of disease severity and disability, researchers reported here.

Spinal Manipulation and Adverse Effects
Unlike pharmacological randomized controlled trials (RCTs), manual-therapy RCTs do not always report adverse events (AEs).
The few manual-therapy randomized controlled trials that provide information on AEs are frequently without details, such as the type and-, severity of the AE and reason for withdrawal.
A recent study was published in the journal Musculoskeletal Science and Practice to prospectively report all adverse events in a chiropractic spinal manipulative therapy (CSMT) randomized controlled trials.
Adverse events were assessed in 703 sessions, with 355 in the chiropractic spinal manipulative therapy group and 348 in the placebo group.
Local tenderness was the most common adverse events, and tiredness on the intervention day was reported.
The study concluded that adverse events were mild and transient, and severe or serious adverse events were not observed.
