Neurotransmitters and the Gut Brain Axis

A study published in the Journal of Cellular Physiology, looked at the role of Neurotransmitters and the Gut Brain Axis.

Neurotransmitters, including catecholamines and serotonin, play a crucial role in maintaining homeostasis in the human body. Studies on these neurotransmitters mainly revolved around their role in the “fight or flight” response, transmitting signals across a chemical synapse and modulating blood flow throughout the body. However, recent research has demonstrated that neurotransmitters can play a significant role in the gastrointestinal (GI) physiology.

Norepinephrine (NE), epinephrine (E), dopamine (DA), and serotonin have recently been a topic of interest because of their roles in the gut physiology and their potential roles in GI and central nervous system pathophysiology. These neurotransmitters are able to regulate and control not only blood flow, but also affect gut motility, nutrient absorption, GI innate immune system, and the microbiome.

Furthermore, in pathological states, such as inflammatory bowel disease (IBD) and Parkinson’s disease, the levels of these neurotransmitters are dysregulated therefore causing a variety of GI symptoms.

Research in this field has shown that exogenous manipulation of catecholamine serum concentrations can help in decreasing symptomology and/or disease progression.

In this review article, the authors discuss the current state-of-the-art research and literature regarding the role of neurotransmitters in regulation of normal GI physiology, their impact on several disease processes, and novel work focused on the use of exogenous hormones and/or psychotropic medications to improve disease symptomology.

Physical exercise as an epigenetic modulator of brain plasticity and cognition

A large amount of evidence has demonstrated the power of exercise to support cognitive function, the effects of which can last for considerable time.

An emerging line of scientific evidence indicates that the effects of exercise are longer lasting than previously thought up to the point to affect future generations. The action of exercise on epigenetic regulation of gene expression seem central to building an “epigenetic memory” to influence long-term brain function and behavior.

In a recent review article, the authors discuss new developments in the epigenetic field connecting exercise with changes in cognitive function, including DNA methylation, histone modifications and microRNAs (miRNAs).

The understanding of how exercise promotes long-term cognitive effects is crucial for directing the power of exercise to reduce the burden of neurological and psychiatric disorders.

Neck Pain and Tactile Deficits

Tactile acuity deficits have been demonstrated in a range of persistent pain conditions and may reflect underlying cortical re-organisation.

A recent Musculoskeletal Science and Practice study aimed to determine whether tactile acuity is impaired in people with chronic neck pain relative to controls, and whether deficits relate to pain location, duration and intensity.

The study concluded that people with chronic neck pain demonstrated tactile acuity deficits in painful and non-painful regions when measured using the two-point discrimination test, with the magnitude of deficits appearing greatest at the neck.

The study also revealed a positive relationship between two-point discrimination and pain intensity/duration, further supporting the main study finding.

MRI Findings and Low Back Pain

The purpose of a recent study published in PLOS One, was to conduct a 10-year longitudinal analysis of the relationship between magnetic resonance imaging (MRI) findings and low back pain (LBP).

Ninety-one volunteers with a history of LBP, but without current LBP were recruited between 2005 and 2006. Participants’ baseline demographics and MRI findings were recorded.

Compared with the group without LBP history, the group that had LBP history during the 10 years did not have a significantly increased prevalence of disc degeneration, disc bulging, and high intensity zone in the follow-up and baseline MRIs. Spondylolisthesis and any type of Modic changes were also not associated with LBP history during the 10 years.

The study concluded that follow-up MRI findings consistent with Pfirrmann grading ≥4, disc bulging, high intensity zone, spondylolisthesis, and any type of Modic changes were not associated with LBP history during the 10 years between the baseline and follow-up study.

The progresses of these findings were also not associated with the LBP history. In addition, baseline MRI findings were not associated with LBP history during the 10 years; therefore, the author’s data suggest that baseline MRI findings cannot predict future LBP.

Final Rules Affecting CMS Payments for 2018

ChiroCode recently sent out a bulletin concerning upcoming coding changes for 2018.

On November 2, 2017, the Centers for Medicare and Medicaid Services, the CMS, announced the finalization of four rules which directly impact the following payment systems:

  1. Physician Fee Schedule Final Policy, Payment, and Quality Provisions for CY 2018
  2. Hospital OPPS and ASC Payment System and Quality Reporting Programs Changes for 2018
  3. HHAs: Payment Changes for 2018
  4. Quality Payment Program Rule for Year 2

Of these four rulings, the physician fee schedule rule and the Quality Payment Program Rule are the most applicable to doctors of chiropractic.

Effects of Cervical Manipulation on Range of Motion, Strength Performance, and Cardiovascular Outcomes

Cervical high-velocity low-amplitude (HVLA) manipulation technique is among the oldest and most frequently used chiropractic manual therapy, but the physiologic and biomechanics effects were not completely clear.

The aim of this review, published in the Journal of Alternative and Complementary Medicine, was to describe the effects of cervical HVLA manipulation techniques on range of motion, strength, and cardiovascular performance.

The review shows that cervical HVLA manipulation treatment results in a large effect size on increasing cervical range of motion and mouth opening.

In patients with lateral epicondylalgia, cervical HVLA manipulation resulted in increased pain-free handgrip strength, with large effect sizes.

Finally, in subjects with hypertension the blood pressure seemed to decrease after cervical HVLA manipulation.

Higher quality studies are needed to develop a stronger evidence-based foundation for HVLA manipulation techniques as a treatment for cervical conditions.

Spinal pain and its impact on older people

The term ‘spinal pain’ collectively includes the cervical, thoracic and lumbosacral regions of the spine.

The majority of older people experience spinal pain, and with an increasing proportion of older people, the prevalence of spinal conditions are expected to increase in the coming decades.

Musculoskeletal conditions of the spine in the older patient commonly include osteoarthritis and spinal stenosis, and the result of these degenerative diseases includes pain, stiffness and a decreased ability to engage in everyday activities.

More than just the burden of pain, spinal pain has a significant considerable impact on the well-being and independence of older people within the community.

Spinal pain is poorly managed, and knowledge of safe and effective treatment strategies are lacking because of the exclusion of older people in clinical research.

Spinal pain in older people is a global health problem; the physical and personal impact of spinal directly threatens efforts to support healthy ageing.

The Effectiveness and Safety of Manual Therapy on Pain and Disability in Older Persons With Chronic Low Back Pain

The aim of this study published in the Journal of Manipulative and Physiological Therapeutics, was to perform a systematic review of the literature of the effectiveness and safety of manual therapy interventions on pain and disability in older persons with chronic low back pain (LBP).

A literature search of 4 electronic databases was performed. Inclusion criteria included randomized controlled trials of manual therapy interventions on older persons who had chronic LBP.

The search identified 405 articles, and 38 full-text articles were assessed.

The study concluded that a limited number of studies have investigated the effectiveness and safety of manual therapy in the management of older people with chronic LBP.

The current evidence to make firm clinical recommendations is limited.

Research with appropriately designed trials to investigate the effectiveness and safety of manual therapy interventions in older persons with chronic LBP is required.
