Health Leaders Support Chiropractic Treatment at Industry Briefing

Chiropractic is a safer first-line option for pain management than prescription opioid painkillers for fibromyalgia patients and others with chronic pain, numerous speakers insisted at a March 21 healthcare industry briefing hosted by the Foundation for Chiropractic Progress (F4CP).

Marc Siegel, MD, Fox News Health commentator and professor of medicine at New York University’s Langone Medical Center, praised the chiropractic profession for its drug-free approach to the management of back and neck pain.

According to Dr. Siegel, “Primary-care doctors are responsible for 28 percent of the problem, and let me tell you why: someone comes to us for back pain, and maybe we understand that the back is a tight compartment of muscle, nerve and bone, but maybe we don’t know which is causing the problem. So, what do primary care doctors do? They start trying to medicate to get the patient feeling better.”

Noninvasive Non-pharmacological Treatment for Chronic Pain

In June 2018, the Agency for Healthcare Research and Quality, AHRQ, released a summary report looking to assess which noninvasive non-pharmacological treatments for common chronic pain conditions improve function and pain for at least 1 month after treatment.

Many interventions are available to manage chronic pain; understanding the durability of treatment effects may assist with treatment selection.

Key Messages from the report included:

  • Interventions that improved function and/or pain for at least 1 month when used for—
    • Chronic low back pain:Exercise, psychological therapies (primarily cognitive behavioral therapy [CBT]), spinal manipulation, low-level laser therapy, massage, mindfulness-based stress reduction, yoga, acupuncture, multidisciplinary rehabilitation (MDR).
    • Chronic neck pain:Exercise, low-level laser, Alexander Technique, acupuncture.
    • Knee osteoarthritis:Exercise, ultrasound.
    • Hip osteoarthritis:Exercise, manual therapies.
    • Fibromyalgia:Exercise, CBT, myofascial release massage, tai chi, qigong, acupuncture, MDR.
    • Chronic tension headache:Spinal manipulation.
  • Most effects were small. Long-term evidence was sparse.
  • There was no evidence suggesting serious harms from any of the interventions studied; data on harms were limited.

NYS Proposed New Workers’ Compensation Fee Schedule

On June 6, 2018, the NYS Workers’ Compensation Board released the following statement:

“As announced on April 17, 2018, in Subject Number 046-1058 Proposals to Improve Medical Care for Injured Workers, the New York State Workers’ Compensation Board (Board) proposes to increase reimbursement fees paid to medical providers.

The proposed changes to the Medical Fee Schedule have been published in the June 6, 2018, State Register, and the Board will accept comments from the public for 60 days.

The proposal includes updated Current Procedural Terminology (CPT) codes, increased conversion factors and amended Ground Rules, which are available under the Medical Fee Schedules section on the Board’s website. Those who do not have access to CPT codes with descriptions can arrange to review them in person at Board offices by contacting regulations@wcb.ny.gov

The Board encourages the public, injured workers, employers, self-insured employers, insurance carriers, third-party administrators, attorneys, medical providers, and labor and business organizations to provide comment.

Please submit your comments on or before August 6, 2018, to regulations@wcb.ny.gov

The Board will evaluate all comments received, and will consider necessary revisions as the process advances.”

Effect of Usual Medical Care Plus Chiropractic Care vs Usual Medical Care Alone on Pain and Disability

A recent study published in JAMA, studied the effect of adding chiropractic care to usual medical care for patients with low back pain.

In this comparative effectiveness clinical trial among active-duty US military personnel, patients who received usual medical care plus chiropractic care reported a statistically significant moderate improvement in low back pain intensity and disability at 6 weeks compared with those who received usual care alone.

This trial supports the inclusion of chiropractic care as a component of multidisciplinary health care for low back pain, as currently recommended in existing guidelines.

The objective of the study was to determine whether the addition of chiropractic care to usual medical care results in better pain relief and pain-related function when compared with usual medical care alone.

The study concluded that Chiropractic care, when added to usual medical care, resulted in moderate short-term improvements in low back pain intensity and disability in active-duty military personnel.

This trial provides additional support for the inclusion of chiropractic care as a component of multidisciplinary health care for low back pain, as currently recommended in existing guidelines.

However, study limitations illustrate that further research is needed to understand longer-term outcomes as well as how patient heterogeneity and intervention variations affect patient responses to chiropractic care.

The impact of low back pain on health-related quality of life in old age

The aim of a recent study published in the European Spine Journal, was to investigate the effects of low back pain (LBP), i.e., type of symptoms, activity limitations, frequency, duration, and severity on health-related quality of life (HRQoL) in a sample of 707 community-dwelling men and women aged ≥ 65 years living in Switzerland.

The study is part of a larger survey conducted in Switzerland on a sample of older adults selected randomly from population records, stratified by age and sex.

The Standardized Back Pain Definition was used to investigate LBP, and HRQoL was assessed by means of the EQ-5D, including Health Utility Index (HUI) measures.

The study concluded that these results provide further insight into the impact of qualitative aspects of LBP and in particular the importance of radiating leg pain and pain frequency and duration.

While LBP-related activity limitations had little impact on both self-rated overall health and HUI, radiating leg pain and pain frequency and duration were associated with significantly decreased scores on both dimensions.

Manipulation and mobilization for treating chronic low back pain

Mobilization and manipulation therapies are widely used to benefit patients with chronic low back pain. However, questions remain about their efficacy, dosing, safety, and how these approaches compare with other therapies.

A recent study, published in The Spine Journal, aims to determine the efficacy, effectiveness, and safety of various mobilization and manipulation therapies for treatment of chronic low back pain.

The present study measures self-reported pain, function, health-related quality of life, and adverse events.

Fifty-one trials were included in the systematic review. Nine trials (1,176 patients) provided sufficient data and were judged similar enough to be pooled for meta-analysis.

Studies comparing manipulation or mobilization with sham or no treatment were too few or too heterogeneous to allow for pooling as were studies examining relationships between dose and outcomes.

Few studies assessed health-related quality of life. Twenty-six of 51 trials were multimodal studies and narratively described.
