Rejected Medicare CMS 1500 Forms

The ANJC in New Jersey recently reported on rejection of Medicare CMS 1500 Forms.

The ANJC reported that they have heard from numerous members who have received rejections from Medicare due to a format issue concerning dates on the CMS 1500 claim submission form.

The NJ regional Medicare Administrative Contractor (MAC), Novitas, began rejecting claims that did not have the proper date formatting as of July 30, 2018.

The article goes on to discuss specific requirements for the Novitas MAC.

Resistance Training Volume Enhances Muscle Hypertrophy

The purpose of a recent study, published in the journal Medicine & Science in Sports & Exercise, was to evaluate muscular adaptations between low-, moderate-, and high-volume resistance training (RT) protocols in resistance-trained men.

Thirty-four healthy resistance-trained men were randomly assigned to 1 of 3 experimental groups: a low-volume group (1SET) performing 1 set per exercise per training session; a moderate-volume group (3SET) performing 3 sets per exercise per training session; or a high-volume group (5SET) performing 5 sets per exercise per training session.

The study concluded that marked increases in strength and endurance can be attained by resistance-trained individuals with just three, 13-minute weekly sessions over an 8-week period, and these gains are similar to that achieved with a substantially greater time commitment.

Alternatively, muscle hypertrophy follows a dose-response relationship, with increasingly greater gains achieved with higher training volumes.

Multifidus muscle size changes at different directions of head and neck movements in females

The aim of a recent study, published in the Journal of Bodywork and Movement Therapies, was to compare the dimensions of cervical multifidus muscle (CMM) in different conditions.

Twenty-five women with neck pain and 25 healthy subjects participated in this study. The dimensions of the cervical multifidus muscle were measured at rest, 50% and 100% maximum isometric voluntary contraction (MIVC) at six directions of neck movements, using ultrasonography.

The study concluded that the size of cervical multifidus muscle was decreased in patients with neck pain in rest state. The size of cervical multifidus muscle changes in all directions of neck movements, although the most prominent was during neck extension.

This points out cervical multifidus muscle stabilization role’s in different directions of neck movements.

Leg press exercise can reduce functional hamstring:quadriceps ratio in the elderly

The aim of a recent study, published in the Journal of Bodywork and Movement Therapies, was to investigate whether 12 weeks of leg press strength training exercise could affect the conventional and functional hamstring:quadriceps ratios in the elderly.

Twelve elderly participants were submitted to a 12 week progressive training protocol (two sessions/week) using a 45° leg press exercise.

A significant increase in the one repetition maximum was observed after 4, 8, and 12 weeks compared to week 0 and after 8 and 12 weeks compared to week 4.

The study concluded that although the 45° leg press exercise requires knee extensor and flexor, hip extensor, and plantar flexor muscle strength, the author’s findings suggest that the isolated use of the 45° leg press exercise reduces the knee functional ratio after 8 weeks of training.

Therefore, 45° leg press exercise alone, without a hamstring exercise, should not be recommended for elderly individuals.

Immediate Effects of Thoracic Spinal Manipulation on Pulmonary Function in Stroke Patients

The purpose of a recent study, published in the journal JMPT, was to investigate the immediate effects of thoracic spinal manipulation (TSM) on pulmonary function in stroke patients.

Pulmonary function test was repeated immediately after the intervention. Forced vital capacity, forced expiratory volume at 1 second, maximum voluntary ventilation, and residual volume were measured by a spirometer in pre-intervention and post-intervention.

The study concluded that the pulmonary function values for patients in the TSM group were significantly enhanced with no significant improvement in maximum voluntary ventilation and residual volume.

Mechanical factors may be responsible for the improved pulmonary function in the TSM group.

Spinal Misalignments and Heart Palpitations

In a recent article discussing causes of Heart Palpitations, Spinal Misalignments are mentioned.

The article focuses on Dr. Martin Gallagher. He’s both an M.D. and a chiropractor, and he’s worked with a number of patients whose irregular heartbeats were improved with chiropractic adjustments.

This isn’t something that tends to get written up in medical literature, but it makes sense. Even slight misalignments in the spine can put pressure on your nerves, which stimulates the sympathetic nervous system to release adrenaline, cortisol, and other stress hormones. And stress, whether physical or emotional, can be a major cause of heart palpitations.

On its own, spinal misalignment isn’t as immediately dangerous as A-fib, MVP, or diastolic dysfunction. But it’s worth noting because the chronic stress it causes drives a lot of inflammation in the body. Too much inflammation speeds up the aging process and raises your risk for heart disease, insulin resistance, and other long-term degenerative conditions.
