Low Muscle Mass and Continuum of Care

Abnormalities in body composition can occur at any body weight. Low muscle mass is a predictor of poor morbidity and mortality and occurs in several populations.

A recent narrative review published in the journal Annals of Medicine, provides an overview of the importance of low muscle mass on health outcomes for patients in inpatient, outpatient and long-term care clinical settings.

Low muscle mass is associated with outcomes such as higher surgical and post-operative complications, longer length of hospital stay, lower physical function, poorer quality of life and shorter survival.

As such, the potential clinical benefits of preventing and reversing this condition are likely to impact patient outcomes and resource utilization/health care costs.

Clinically viable tools to measure body composition are needed for routine screening and intervention.

Future research studies should elucidate the effectiveness of multimodal interventions to counteract low muscle mass for optimal patient outcomes across the healthcare continuum.

Risk factors for low back pain and sciatica

Low back pain (LBP) is a highly prevalent condition and it is associated with significant disability and work absenteeism worldwide.

A variety of environmental and individual characteristics have been reported to increase the risk of LBP.

According to the Author’s, there has been no previous attempt to summarize the evidence from existing systematic reviews of risk factors for LBP or sciatica.

A recent study was published in the Spine Journal, to provide an overview of risk factors for LBP, we completed an umbrella review of the evidence from existing systematic reviews.

An umbrella review was carried out. A systematic literature search was conducted in MEDLINE, EMBASE, PubMed PsychINFO, and CINAHL databases.

The study concluded that poor general health, physical and psychological stress, and characteristics of the person increase risk for a future episode of LBP or sciatica.

ACA Introduces New Medicare Legislation

Legislation championed by the American Chiropractic Association (ACA) to expand Medicare coverage of services provided by chiropractors within the scope of their license has been introduced in the U.S. House of Representatives.

The bill, H.R. 7157, introduced on Nov. 16th, would allow Medicare beneficiaries to access the chiropractic profession’s broad-based, non-drug approach to pain management, which includes manual manipulation of the spine and extremities, joint mobilization, soft-tissue massage techniques, physiological therapies, exercise instruction and activity advice.

Access to non-drug approaches to pain management in Medicare as well as private health care plans has become increasingly important in the wake of the nationwide opioid crisis. The bill adds no new services; it only allows access to those current Medicare benefits that chiropractors are licensed to provide.

Since 1972, the year chiropractic was first made available in the Medicare program, until today, Medicare beneficiaries have been covered for only one chiropractic service—manual manipulation of the spine—forcing them to access additional medically necessary care from other types of providers or to pay out of pocket for the services from their chiropractor.

Chiropractors are the only physician-level providers in the Medicare program whose services are restricted in this manner.

ACA Year to Date Update

The American Chiropractic Association (ACA) recently sent out a mailer with their Year to Date Update, as of November 2018.

Following are some of the highlights:

In January 2018, President Trump signed into law the ACA-supported “Jobs for Our Heroes Act.” The law includes a provision allowing chiropractors working within the U.S. Department of Veterans Affairs to perform physical exams on veterans needing a medical certificate to operate a commercial motor vehicle.

In February 2018, ACA was successful in getting “The Chiropractic Health Parity for Military Beneficiaries Act” introduced in the U.S. House of Representatives. The bill would expand access to chiropractic to military retirees, dependents and survivors through the U.S. Department of Defense TRICARE program.

In March 2018, President Trump signed into law an ACA-championed plan to expand access to chiropractic to American veterans. The approved language calls for the U.S. Department of Veterans Affairs (VA) to expand the availability of chiropractic services at no fewer than two medical centers or clinics in each Veterans Integrated Service Network (VISN) by no later than Dec. 31, 2019, and at no fewer than 50 percent of all medical centers in each VISN by no later than Dec. 31, 2021.

Currently, veterans have access to chiropractic services at more than 70 major VA treatment facilities in the United States–but nearly 100 VA sites offer little to no availability.

In July 2018, ACA rallied chiropractic organizations from across the country to opposes a new policy by United Healthcare (UHC) that denied reimbursement for spinal manipulative therapy for the treatment of headache.

ACA drafted a letter to the UHC President and CEO that eventually included the names of leaders from 41 national, state and regional chiropractic associations. This effort was pulled together in just a few days after the UHC policy was announced.

Finally, in October 2018, the president signed into law ACA-supported legislation that protects chiropractors who travel with sports teams by ensuring that their license and liability insurance remains in effect even when they cross state lines.

Thanks to ACA, the Sports Medicine Licensure Clarity Act of 2018 was included in a larger, unrelated piece of legislation (R. 302) that passed the Senate overwhelmingly on Oct. 3rd.

The original version may have excluded chiropractors from the list of included “sports medicine professionals.”

Updated Nutritional Supplement Unit Conversion Factors

In May 2016, the U.S. Food and Drug Administration (FDA) announced regulations that require amendments to the existing supplement facts label, which uses units and conversions based on the 1968 Recommended Daily Allowances (RDA).

The new regulations will be mandatory in 2019-2020. Voluntary compliance prior to that date is permitted, and it is expected that there will be a period of time when both types of labels will be on the market.

The changes that will affect the application of DSID results are the changes in the units used to declare vitamins and minerals on supplement labels.

The FDA will permit manufacturers to include the amounts of nutrients in the old units in parentheses adjacent to the amounts in the new units.

For example, when looking at Vitamin A units, presently designated in IU measurements, the updated Unit of Measurement will be listed as mcg RAE, the latter referring to Retinol Activity Equivalent.

Effect of Exercise Therapy on Inflammation Biomarkers

A recent study was published in the journal Arthritis Care and Research to investigate the impact of exercise therapy on molecular biomarkers related to cartilage and inflammation in people at risk of, or with established, knee osteoarthritis by conducting a systematic review of randomized controlled trials (RCTs).

Literature search up to September 2017 in five major databases with no restriction on publication year or language.

The study concluded that exercise therapy is not harmful, as it does not increase the concentration of molecular biomarkers related to cartilage turnover and inflammation, implicated in osteoarthritis progression.

The overall quality of evidence was downgraded to low because of the limited number of RCTs available.

Chiropractors and Limiting Opioid Treatments

A recent study in the journal Mayo Clinic Proceedings, was published to examine patients seeking care for neck pain to determine associations between the type of provider initially consulted and 1-year health care utilization.

A retrospective cohort of 1702 patients (69.25% women, average age, 45.32±14.75 years) with a new episode of neck pain who consulted a primary care provider, physical therapist (PT), chiropractor (DC), or specialist from January 1, 2012, to June 30, 2013, was analyzed. Descriptive statistics were calculated for each group, and subsequent 1-year health care utilization of imaging, opioids, surgery, and injections was compared between groups.

Compared with initial primary care provider consultation, patients consulting with a DC or PT had decreased odds of being prescribed opioids within 1 year from the index visit.

The study concluded that initially consulting with a non-pharmacological provider may decrease opioid exposure (PT and DC) over the next year and also decrease advanced imaging and injections (DC only).

These data provide an initial indication of how following recent practice guidelines may influence health care utilization in patients with a new episode of neck pain.

Sexual activity after spine surgery

Sexual function is an important determinant of quality of life, and factors such as surgical approach, performance of fusion, neurological function and residual pain can affect it after spine surgery.

The aim of a recent study published in the European Spine Journal, was to perform a systematic review to collate evidence regarding the impact of spine surgery on sexual function.

A systematic review of studies reporting measures of sexual function, and incidence of adverse sexual outcomes after major spine surgery was done, regardless of spinal location.

The study conclude that despite limited evidence from high-quality articles, there is a general trend towards improvement of sexual activity and function after spine surgery.

Future studies incorporating specific assessments of sexual activity will be required to address this important determinant of quality of life so that appropriate pre-operative counseling can be done by providers.
