Inactivity and Children

Physical activity has many health benefits for young people. In 2018, WHO launched More Active People for a Healthier World, a new global action on physical activity, including new targets of a 15% relative reduction of global prevalence of insufficient physical activity by 2030 among adolescents and adults.

The authors of a recent review of data published in The Lancet,  describe current prevalence and trends of insufficient physical activity among school-going adolescents aged 11–17 years by country, region, and globally.

The majority of adolescents do not meet current physical activity guidelines. Urgent scaling up of implementation of known effective policies and programmes is needed to increase activity in adolescents.

Investment and leadership at all levels to intervene on the multiple causes and inequities that might perpetuate the low participation in physical activity and sex differences, as well as engagement of youth themselves, will be vital to strengthen the opportunities for physical activity in all communities.

Such action will improve the health of this and future young generations and support achieving the 2030 Sustainable Development Goals.

Chiropractic and Maintenance Care

Maintenance Care is a traditional chiropractic approach, whereby patients continue treatment after optimum benefit is reached.

A review conducted in 1996 concluded that evidence behind this therapeutic strategy was lacking, and a second review from 2008 reached the same conclusion.

Since then, a systematic research program in the Nordic countries was undertaken to uncover the definition, indications, prevalence of use and beliefs regarding Maintenance Care to make it possible to investigate its clinical usefulness and cost-effectiveness.

As a result, an evidence-based clinical study could be performed. It was therefore timely to review the evidence.

The conclusion of this current review, published in the journal Chiropractic and Manual Therapies, found that knowledge of chiropractic Maintenance Care has advanced. There is reasonable consensus among chiropractors on what Maintenance Care is, how it should be used, and its indications.

Presently, Maintenance Care can be considered an evidence-based method to perform secondary or tertiary prevention in patients with previous episodes of low back pain, who report a good outcome from the initial treatments.

However, these results should not be interpreted as an indication for Maintenance Care on all patients, who receive chiropractic treatment.

Back Complaints in the Elders

Low back pain is a common condition among older adults that significantly influences physical function and participation.

Compared to their younger counterparts, there is limited information available about the clinical course of low back pain in older people, in particularly those presenting for chiropractic care.

Improving our understanding of this patient population and the course of their low back pain may provide input for studies researching safer and more effective care than is currently provided.

The primary objectives of an upcoming study in journal Europe PMC, are to examine the clinical course over one year of the intensity, healthcare costs and improvement rates of low back pain in people 55 years and older who visit a chiropractor for a new episode of low back pain.

Patients 55 years and older who are accepted for care having presented to a chiropractor with a new episode of low back pain and who have not been to a chiropractor in the previous six months for an episode of low back pain are to be included, independent of whether or not they have seen another type of health care provider.

Back Pain Triggers in Children

Back pain is a global problem in terms of disability and financially, with a large burden both to the individual and to society. Back pain was previously believed to be uncommon in children. However, there is a growing body of evidence that this is not the case.

A recent review published in the journal Chiropractic & Manual Therapies, studied risk factors of incident and episodic back pain. In part II of this review, the authors aimed to identify all risk factors and triggers with unclear or mixed type back pain in young people and to identify any gaps in the literature.

The review concluded that many of the included studies approached risk factors in similar ways and found factors that were associated with back pain but were not obvious risk factors as causality was uncertain.

Future research should be more rigorous and innovative in the way that risk factors are considered. This could be through statistical approaches including cumulative exposures, or longitudinal approaches including multi-trajectory methods. Additionally, data on proposed risk factors should be collected before the onset of back pain.

Conservative LBP Therapy and Opioid Use

A recent study published in BMJ Open examined the association of initial provider treatment with early and long-term opioid use in a national sample of patients with new-onset low back pain (LBP).

The study evaluated outpatient and inpatient claims from patient visits, pharmacy claims and inpatient and outpatient procedures with initial providers seen for new-onset LBP.

This study assesses the impact of state regulations of access to physical therapy on choice of initial provider.

The study concluded that initial visits to chiropractors or physical therapists is associated with substantially decreased early and long-term use of opioids. Incentivizing use of conservative therapists may be a strategy to reduce risks of early and long-term opioid use.

Chronic LBP and Tx Effectiveness

Chronic non-specific low back pain (CLBP) is a common clinical condition that has impacts at both the individual and societal level.

Pain intensity is a primary outcome used in clinical practice to quantify the severity of CLBP and the efficacy of its treatment, however, pain is a subjective experience that is impacted by a multitude of factors.

Moreover, differences in effect sizes for pain intensity are not observed between common conservative treatments, such as spinal manipulative therapy, cognitive behavioural therapy, acupuncture and exercise training.

As pain science evolves, the biopsychosocial model is gaining interest in its application for CLBP management.

The aim of a recent paper published in the journal Pain Practice, was to discuss the current scientific understanding of pain and present why additional factors should be considered in conservative CLBP management.

This information can guide treatment selection when working with an individual with CLBP.
