Preoperative symptom duration and Lumbar Spinal Stenosis

Lumbar degenerative stenosis is one of the most common spine pathologies for which surgical intervention is indicated. There is some evidence that a prolonged duration of neurological compression could lead to a failure of surgery to alleviate symptoms.

The purpose of a recent study published in The Spinal Journal, was determination of whether longer symptom duration was associated with worse postoperative disability outcomes after decompressive surgery for lumbar degenerative stenosis.

Change between preoperative and 12-month postoperative Oswestry Disability Index (ODI) was compared between symptom duration groups. Secondary outcomes included SF12 physical component score (PCS), and numeric rating scales for leg and back pain.

The study concluded that multicenter registry data provides important real-world evidence to guide consent, surgical planning, and health resource management.

Longer symptom duration was found to correlate with less improvement in pain and disability after lumbar stenosis surgery suggesting that these patients may benefit from earlier treatment.

New City Chiropractic Center Collects Hundreds of Toys

And keeping in the Holiday Spirit, we finish this week about a story from New York, in the US.

New City Chiropractic Center was established in 1986 and set up a Toys for Tots donation center in their office that year. Over the past 33 years they have collected over 4,000 toys to donate to Toys for Tots.

Patients bring in toys throughout the holiday season and this year ConnectOneBank, with offices in Bardonia and Blauvelt, helped out by contributing several bags filled with toys.

In addition to serving thousands of patients over the past three decades, New City Chiropractic Center has been a leader in the community in many charitable ways.

They have dedicated their time and energy to numerous organizations including the annual Rockland Road Runners Turkey Trot Race, the MS Society, Wounded Warriors, Home Aides of Rockland, Blauvelt Sons of Italy, Keep Rockland Beautiful, and BRIDGES.

Deformations of abdominal muscles under experimentally induced low back pain

Low back pain (LBP) is associated with altered motor control and muscle activity; however, it remains unknown whether these changes predispose humans to injury and pain or are the consequence of ongoing nociceptive processes.

In a recent experimental study, published in the European Spine Journal, the authors aimed to use a novel ultrasound imaging technique for the measurement of lateral abdominal wall muscle activity: the tissue deformation index (TDI).

Experimentally induced LBP suppresses deformation of the superficial abdominal muscles and increases the variability of local/deep muscles.

Further research is required to confirm these observations.

Predicting a beneficial response to motor control training in patients with LBP

Exercise therapy such as motor control training (MCT) has been shown to reduce pain and disability in people with low back pain (LBP). It is unknown which patients are most likely to benefit.

A recent longitudinal cohort study, published in the European Spine Journal, aimed to:

(1) retrospectively examine records from a large cohort of patients who received motor control training treatment

(2) identify potentially important predictors of response to motor control training and

(3) test the predictors on an independent (split) sample derived from the original cohort of patients, using one group to identify the predictors and the other to test them.

The conclusion found that this study provides a first step towards assisting clinicians to select patients most likely to respond to motor control training.
