Medicare and Acupuncture Coverage

A recent ChiroCode email detailed Medicare’s updated policy related to Acupuncture coverage.

On January 20, 2020, Medicare announced that they would begin covering acupuncture for the treatment of chronic low back pain.

However, the way that it is currently worded, a chiropractor cannot ‘order’ acupuncture, BUT they may provide the services as an auxiliary provider as long as it is ordered by a physician meeting their requirements.

This all goes back to the original problem of Medicare’s definition of a physician and how they define a doctor of chiropractic.

Chiropractors who are ACAOM certified and licensed in their state to provide acupuncture may wish to take this opportunity to work with other physicians and provide these services under the ‘Auxiliary personnel’ provision of this National Coverage Determination (NCD).

It is recommended to touch base with your State organization for full details of what is and is not permitted.

Vitamin D and Lumbar Disc Degeneration

A recent study was published in the journal Menopause, to investigate the relationship between serum vitamin D concentration and lumbar disc degeneration (LDD) in postmenopausal women and the epidemiologic factors affecting low back pain (LBP).

Between July 2017 and December 2018, 232 participants were retrospectively enrolled. Serum concentrations of bone turnover markers were measured using electrochemiluminescence assays. Disc degeneration was evaluated using the Pfirrmann grading system.

The study concluded that Vitamin D deficiency is associated with LDD and LBP in postmenopausal women. Specifically, a serum vitamin D concentration < 10 ng/mL is a marker of severe LDD and LBP.

Smoking, severe vitamin D deficiency, lack of vitamin D supplementation, high body mass index, and osteoporosis are associated with a higher prevalence of moderate-to-severe pain.

Medicare and Acupuncture Coverage

Recently, the Centers for Medicare & Medicaid Services (CMS) finalized a decision to cover acupuncture for Medicare patients with chronic low back pain. Before this final National Coverage Determination (NCD) reconsideration, acupuncture was nationally non-covered by Medicare. CMS conducted evidence reviews and examined the coverage policies of private payers to inform today’s decision.

Chiropractic Integration into Private Sector Facilities

Chiropractic care may have value in improving patient outcomes and decreasing opioid use, but little is known about the impetus for or process of incorporating these services into conventional medical settings.

A 2018 study was published in the Journal of Alternative and Complementary Medicine to describe organizational structures, care processes, and perceived value of chiropractic integration within U.S. private sector medical facilities.

Delivery of chiropractic services was perceived to have high value among patients, medical providers, and administration. Patient clinical outcomes, patient satisfaction, provider productivity, and cost offset were identified as markers of clinic success.

The study concluded that a diverse group of U.S. private sector medical facilities have implemented chiropractic clinics, and a wide variety of facility stakeholders report high satisfaction with the care provided.

Whiplash Evaluation of Eye Head and Trunk Coordination

Motor vehicle accidents (MVA) are the most common causes of whiplash injuries. Difficulties with driving and changes in driving behavior are reported by subjects with chronic whiplash associated disorders (WAD).

Proper eye and head coordination is required for driving tasks. Disturbances of eye an head coordination were found in these subjects with chronic WAD.

The objective of a recent pilot study, published in Musculoskeletal Science and Practice, was to evaluate eye, head and trunk coordination in subjects with chronic WAD due to MVA and healthy controls during a target-tracking task using a functionally oriented approach in the context of driving.

This study shows indications of impairments of eye and head coordination in chronic WAD due to MVA when compared to healthy subjects. These alterations may have implications for driving safety.

Physical Activity and Exercise in Youth Mental Health

A recent scoping review, published in BMJ Open Sport & Exercise Medicine, examined the breadth and outcomes of controlled trials testing the effect of physical activity and exercise interventions across all mental health outcomes for mental health promotion and indicated prevention studies in young people.

Thirty publications were included. Available evidence suggested that interventions of varying intensity may lead to a reduction in depression symptoms and that moderate-to-vigorous-intensity and light-intensity interventions may reduce anxiety symptoms.

Effects of physical activity/exercise interventions on additional mental health outcomes were also shown; however, the number of studies was small, indicating a limited evidence base.

The review concluded that available evidence suggests that physical activity/exercise is a promising mental health promotion and early intervention strategy and warrants further investigation.

Chiro Patient Coping Techniques for Chronic LBP

The purpose of a recent study published in JMPT, was to describe coping strategies (eg, mechanisms, including self-treatment) that a person uses to reduce pain and its impact on functioning as reported by patients with chronic low back pain who were seen by doctors of chiropractic and how these coping strategies vary by patient characteristics.

Data were collected from a national sample of US chiropractic patients recruited from chiropractic practices in 6 states from major geographical regions of the United States using a multistage stratified sampling strategy.

A total of 1677 respondents with chronic low back pain reported using an average of 9 coping behaviors in the prior 6 months.

Exercise was more frequent among the healthy and those with less pain. Female respondents tended to report using more coping behaviors than men, and Hispanics more than non-Hispanics.

The study concluded that persons with chronic back pain were proactive in their coping strategies and frequently used self-care coping strategies like those provided by chiropractors in patient education.

In alignment with patients’ beliefs that their condition was chronic and lifelong, many patients attempted a wide range of coping strategies to relieve their pain.
