Lumbar SMT and Pain Sensitivity

According to the American Physical Therapy Association, there is strong evidence to show that vertebral mobilization and manipulation procedures can be used to improve spinal and hip mobility and reduce pain and incapacity in low back pain patients that fit the clinical prediction rule.

The objectives of a recent study published in the journal Chiropractic & Manual Therapies, was to evaluate the immediate effects of high-velocity low-amplitude (HVLA) manipulation on pain and postural control parameters in individuals with nonspecific low back pain.

The study concluded that there was a reduction in subjective pain intensity, evaluated using a numerical scale, in both the intervention and control groups immediately after the intervention, suggesting that the spinal manipulation had a similar effect to the placebo procedure.

No effect of HVLA lumbar manipulation was identified for postural control variables in either the intervention or control groups.

Chiropractic and Treating Patients with Lumbar Fusion

Little is known about the preferred treatment strategies of chiropractors in managing low back pain patients with prior lumbar fusions.

There are several case reports which describe chiropractic care following surgical intervention, but there are no cohort or experimental studies published.

Therefore, the authors of a recent survey, published in the journal Chiropractic & Manual Therapies, sought to examine self-reported management approaches and practice patterns related to the management of patients with prior surgical lumbar fusion, among United States Veterans Affairs (VA) chiropractors.

The findings found that this survey provides preliminary data on VA chiropractic services in the management of patients with prior lumbar fusion.

These patients are often seen by VA chiropractors, and the author’s findings support the need for further study to advance understanding of interventions utilized by chiropractors in this patient population.

Covid-19 and Telehealth

A recent mailing by ChiroCode, stressed the need to keep in touch with the various insurance coverages related to conducting Telehealth patient visits.  According to ChiroCode:

“As our country moves forward with a phased approach to reopening, be sure to pay close attention to individual payer policies regarding how long these changes will remain in effect. Keep in mind that private payer, federal programs (Medicare, Medicaid), and Medicare Advantage plans can all have different timelines as well as different coverage.

For example, on May 31, 2020, United Healthcare updated their COVID information page to reflect some policy changes. They will reimburse doctors of chiropractic for various services when rendered using live video-conferencing while a patient is at home:

  • E/M office/outpatient visit
  • Therapeutic procedures
  • Tests and measurements
  • Orthotic/prosthetic management/training

The dates for these services vary depending on the plan:

  • Medicare Advantage: March 18, 2020 through Sept. 30, 2020
  • Medicaid: Depends on the state
  • Individual and fully insured group market health plans: March 18, 2020 through July 24, 2020”

Lumbar Lordosis, Sitting and Standing

A recent study in the Journal of Manipulative and Physiological Therapeutics, had the aim to evaluate lumbar lordosis during sit-to-stand (STS) and stand-to-sit (SIT) in individuals with and without chronic nonspecific low back pain (CNLBP). T

The second objective was to investigate sex-related differences in lumbar lordosis.

The study found that lumbar lordosis was decreased in patients with CNLBP during sit-to-stand and stand-to-sit compared with the asymptomatic group. Furthermore, no differences were seen in lumbar lordosis at starting position between CNLBP and asymptomatic groups during sit-to-stand and stand-to-sit.

The study concluded that decreased lumbar lordosis in patients with CNLBP during sit-to-stand and stand-to-sit could be considered as an important point during rehabilitation.

Moreover, the present study showed that there is a sex-related difference among women and men in lumbar lordosis during sit-to-stand and stand-to-sit tasks.

Motivational Interviewing in Pain Neuroscience Education for People With Chronic Pain

A study in the journal Physical Therapy, looked at Motivational Interviewing and Pain Neuroscience Education for People with Chronic Pain.

Pain neuroscience education (PNE) and motivational interviewing (MI) have been widely implemented and tested in the field of chronic pain management, and both strategies have been shown to be effective in the short term (small effect sizes) for the management of chronic pain.

Pain neuroscience education uses contemporary pain science to educate patients about the biopsychosocial nature of the chronicity of their pain experience. The goal of Pain neuroscience education is to optimize patients’ pain beliefs/perceptions to facilitate the acquisition of adaptive pain–coping strategies.

Motivational interviewing, on the other hand, is a patient-centered communication style for eliciting and enhancing motivation for behavior change by shifting the patient away from a state of indecision or uncertainty.

Conceptually, Pain neuroscience education and motivational interviewing appear to be complementary interventions, with complementary rather than overlapping effects; motivational interviewing primarily improves cognitive and behavioral awareness and, potentially, adherence to treatment principles, whereas Pain neuroscience education potentially increases pain knowledge/beliefs, awareness, and willingness to explore psychological factors that are potentially associated with pain.

Therefore, combining Pain neuroscience education with motivational interviewing might lead to improved outcomes with larger and longer-lasting effect sizes.

The combined use of Pain neuroscience education and motivational interviewing in patients having chronic pain is introduced here, along with a description of how clinicians might be able to integrate Pain neuroscience education and motivational interviewing in the treatment of patients experiencing chronic pain.

Clinical trials are needed to examine whether combining PNE with MI is superior to Pain neuroscience education or MI alone for improving pain and quality of life in patients having chronic pain.
