SMT and Brain Neurometabolites in Chronic LBP

In patients with chronic nonspecific low back pain (NCLBP), brain function changes occur due to the neuroplastic changes in different regions.

A recent study, published in the Irish Journal of Medical Science, aimed to evaluate the brain metabolite changes after spinal manipulation, using proton magnetic resonance spectroscopy (1H-MRS).

Patients were randomly assigned to lumbopelvic manipulation or sham.

After treatment, severity of pain and functional disability were significantly reduced in the treatment group vs. sham group).

After treatment, N-acetyl aspartate (NAA) in thalamus, insula, dorsolateral prefrontal cortex (DLPFC) regions, as well as choline (Cho) in the thalamus, insula, and somatosensory cortex (SSC) regions, had increased significantly in the treatment group compared with the sham group.

The study concluded that in the patient with low back pain, spinal manipulation affects the central nervous system and changes the brain metabolites. Consequently, pain and functional disability are reduced.

Osteoporosis and High Intensity Resistance and Impact Training

Optimal osteogenic mechanical loading requires the application of high‐magnitude strains at high rates. High‐intensity resistance and impact training (HiRIT) applies such loads but is not traditionally recommended for individuals with osteoporosis because of a perceived high risk of fracture.

The primary aim of the Lifting Intervention for Training Muscle and Osteoporosis Rehabilitation (LIFTMOR) trial was to determine the efficacy of brief, bone‐targeted HiRIT for improving Femoral Neck (FN) and Lumbar Spine (LS) BMD in postmenopausal women with low to very low bone mass.

Postmenopausal women with low bone mass were recruited and randomized to either 8 months of twice‐weekly, 30‐minute, supervised HiRIT (5 sets of 5 repetitions, >85% 1 repetition maximum) or a home‐based, low‐intensity exercise program.

The study concluded that this brief HiRIT program enhances indices of bone strength and functional performance in postmenopausal women with low bone mass. Contrary to current opinion, HiRIT was efficacious and induced no adverse events under highly supervised conditions for our sample of otherwise healthy postmenopausal women with low to very low bone mass.
