WFC releases new guiding principles document

The World Federation of Chiropractic (WFC) has recently published a suite of guiding principles that will support its strategy for supporting, empowering, promoting and advancing chiropractic globally.

The WFC’s new Principles document consists of 20 statements that set out clearly what the organization stands for and how it views chiropractic as a contemporary global health profession.

The statements, which align with the WFC’s mission to advance awareness, utilization and integration of chiropractic internationally, were approved unanimously by the Board of Directors.

In drafting the statements the WFC has been mindful of the social determinants of health that influence access to care and the variety of environments in which chiropractic is practiced.

Secretary-General Dr Richard Brown commented: “The WFC is an inclusive, global organization and we want these statements to resonate with chiropractors around the world.

They reflect our commitment to support and develop the profession for the benefit of patients and the public and we’re excited to be launching the Principles document at this time.”

Experiences With

Chiropractic Care for Patients With Low Back or Neck Pain

Musculoskeletal disorders are the second leading cause of disability worldwide.

A recent study published in the Journal of Patient Experience, examined experiences of chiropractic patients in the United States with chronic low back or neck pain.

Observational study of 1853 chronic low back pain and neck pain patients (74% female) who completed an online questionnaire at the 3-month follow-up that included Consumer Assessment of Healthcare Providers and Systems (CAHPS) items assessing their experiences with care.

The study concluded that the positive experiences of patients with chronic back and neck pain are supportive of their use of chiropractic care.

Medicare moving forward with planned E/M office visit changes

Major modifications are coming to the coding, documentation, and payment of evaluation and management (E/M) services for office visits as Medicare has signaled its intention to implement finalized guidelines and payment rates as planned on January 1, 2021.

The AMA recent release of the proposed 2021 Medicare physician payment schedule reconfirmed the designated implementation date announced last November by the Centers for Medicare & Medicaid Services (CMS).

Between July 2018 and July 2019, the AMA worked with CMS and convened specialty societies and other health professionals to simplify and streamline the coding and documentation for E/M office visits, making them clinically relevant, and reducing excessive administrative burden.

Key elements of the E/M office visit overhaul include:

Eliminating history and physical exam as elements for code selection. While significant to both visit time and medical decision-making, these elements alone should not determine a visit’s code level.

Allowing physicians to choose whether their documentation is based on medical decision-making (MDM) or total time. This builds on the movement to better recognize the work involved in non-face-to-face services like care coordination.

Modifying MDM criteria to move away from simply adding up tasks to focus on tasks that affect the management of a patient’s condition.

Water Composition on Flavor in Drinking Green and Black Tea

Tea is made from the processed leaves of the Camellia sinensis plant, which is a tropical and subtropical evergreen plant native to Asia. Behind water, tea is the most consumed beverage in the world.

Factors that affect tea brewing include brewing temperature, vessel, and time, water-to-leaf ratio, and, in some reports, the composition of the water used.

In a recent project, published in the journal Nutrients, the authors tested if the water used to brew tea was sufficient to influence perceived flavor to the everyday tea drinker.

Black and green tea were brewed with bottled, tap, and deionized water, with brewing temperature, vessel, time, and the water-to-leaf ratio matched.

The samples were analyzed for color, turbidity, and Epigallocatechin Gallate (EGCG) content.

Results showed that the type of water used to brew tea drastically affected sensory properties of green tea (and mildly also for black tea), which was likely driven by a much greater degree of extraction of bitter catechins in teas brewed with more purified bottled or deionized water.

For the everyday tea drinker who drinks green tea for health, the capability to double the EGCG content in tea by simply brewing with bottled or deionized water represents a clear advantage.

Conversely, those drinking tea for flavor may benefit from instead brewing tea with tap water.

Mechanisms and Origins of Spinal Pain

Spinal pain, especially low back pain (LBP), is a widespread clinical and diagnostic problem for both patients and physicians, because back pain has an equivalently wide variety of causes and provocations.

Because of its variable nature and manifestations, back pain is challenging to diagnose and treat correctly. In addition, the pain is induced not only by direct mechanical pressure such as a herniated disk or degenerated bone but also by inflammation and associated proinflammatory cytokines.

The authors of a recent review explored the microscopic and macroscopic mechanisms for each category of LBP and depict the relationship between imaging findings and pain mechanisms.

This review focuses on the detailed anatomy related to the pain-signaling pathway and the roles of chemicals in inducing different mechanisms of LBP.

MRI findings that serve as representative examples of key concepts are demonstrated according to each mechanism, and treatment options are reviewed on the basis of different causes of LBP.

By knowing these concepts, radiologists can help correlate imaging findings with potential underlying mechanisms and help guide clinicians in the management of LBP.

Sex Specific Sacroiliac Joint Biomechanics During Standing

The comparison of sacroiliac joint (SIJ) angular motions, pelvis ligaments strain, load sharing, and stress distribution across the joint for male and female spine-pelvis-femur models using finite element analysis.

A recent study in the journal Spine, was published to quantify biomechanical parameters at SIJ for all motions for both male and female models.

SIJ has been recognized as a main source of pain in 13% to 30% of patients with low back pain. It is shown that the SIJ rotation and translation in different planes are not exceeding 2° to 3° and 2 mm, respectively.

The study concluded that the female SIJ had higher mobility, stresses, loads, and pelvis ligament strains compared with the male SIJ which led to higher stress across the joint, especially on the sacrum under identical loading conditions.

This could be a possible reason for higher incidence of SIJ pain and pelvic stress fracture in females.
