Back Pain and Work Factors

Gwenllian Wynne-Jones, MD, and colleagues analyzed 11 general practitioners from an existing general practice survey and six physiotherapists selected randomly using “snowball” sampling techniques. Each participant was given a 30-minute interview, and the researchers identified themes contrasted between and within both groups of respondents.

Several themes were identified from the interviews, including approaches to evaluating patients’ work problems, perceived ability to manage work and pain, and policies and penalties in the workplace.

The researchers noted that whereas physiotherapists were more likely to take a structured approach and ask patients about their job and work difficulties, general practitioners were less likely to ask about their patients’ work situations. Additionally, assessing patients’ ability to return to work appeared to cause tension between general practitioners’ roles as “gatekeeper” and patient advocate.

Wynne-Jones and colleagues concluded these health care professionals need to take patients’ work difficulties and their own perceived ability to offer effective guidance in account when working with them. Additionally, to ensure a smooth return to work, practitioners should take into account their patients’ workplace receptivity to their work problems.

Testing for Rotator Cuff Shoulder Injuries

Study data have shown no one test can accurately diagnose rotator cuff tears or tendinosis.

Researchers reviewed 139 consecutive patients being examined at two orthopedic clinics for possible rotator cuff lesions. If a diagnosis was uncertain after taking patients’ medical history, a clinician performed physical examinations to diagnose possible lesions.

Arthroscopy and MRI with arthrogram were the reference standards for patients with and without surgery, respectively. Sensitivity, specificity and likelihood ratios were determined to ascertain whether multiple combinations of frequently used tests offered stronger predictions of accurate diagnosis.

Separately, none of the tests were highly sensitive in the diagnosis of rotator cuff tears or tendinosis, according to the researchers.

Additionally, although no combination of tests improved diagnosis sensitivity and specificity, if there was at least one positive test for tendinosis, sensitivity for detecting full-thickness tears was 88.5%.

Internal rotation and lateral rotation lag sign did not improve the likelihood of an accurate diagnosis of subscapularis or supraspinatus tears; however, lateral rotation lag sign was found to demonstrate a discriminatory ability for tear size.

The ideal cutoff for accurately diagnosing a supraspinatus tear was found to be 2.9 cm with a sensitivity of 80% and a specificity of 81.4%, according to the researchers. Subscapularis tests were unable to differentiate tear sizes.

Platelet Rich Plasma Injections for Hamstring Injuries

A multicenter, double-blind, randomized, placebo-controlled trial examining the use of platelet-rich plasma injections in athletes for the treatment of acute hamstring injuries has shown the injections were no more effective than placebo.

The authors of the study used a two-injection protocol where the first injection of platelet-rich plasma (PRP) or placebo was given 5 days post-injury and a second injection was given 5 to 7 days later.

The median time to return to play for both groups was 42 days.

Furthermore, re-injury rates were also similar for athletes in both the PRP and placebo groups in this study, according to the lead researcher.

Chiropractic Response to Statement on Neck Manipulation and Stroke

On Aug. 7, The American Heart Association and American Stroke Association posted a statement online headlined, “Neck manipulation may be associated with stroke.”

According to the Palmer Center for Chiropractic Research – the world’s largest chiropractic research center – the statement strongly implies a causal relationship between Chiropractic Manipulative Therapy and stroke – which isn’t substantiated by the best research available.

In an Aug. 8 speech, Christine Goertz, D.C., Ph.D., vice chancellor for research and health policy at Palmer College of Chiropractic, explained that medical doctors and doctors of chiropractic need to be vigilant in assessing patients who may be in the early stages of Vertebral Arterial Dissection (VAD). And it’s also extremely important that the data regarding the risk of VAD is presented to patients in an accurate manner.

“The facts are that VADs are very, very rare events, and there’s absolutely no research that shows a cause-and-effect relationship between chiropractic care and stroke,” said Dr. Goertz. “Doctors need to be careful about how they counsel patients based on misleading statements, like this one from the American Heart Association.”

Health and Human Services Announces ICD-10 Implementation Date

The US Department of Health and Human Services issued a final rule in the Federal Register on July 31, firmly establishing October 1, 2015 as the official compliance date for providers, health plans, and clearinghouses to transition to ICD-10.

This final rule implements section 212 of the Protecting Access to Medicare Act of 2014 by changing the compliance date for the International Classification of Diseases, 10th Revision, Clinical Modification (ICD-10-CM) for diagnosis coding, including the Official ICD-10-CM Guidelines for Coding and Reporting, and the International Classification of Diseases, 10th Revision, Procedure Coding System (ICD-10-PCS) for inpatient hospital procedure coding, including the Official ICD-10-PCS Guidelines for Coding and Reporting, from October 1, 2014 to October 1, 2015.

It also requires the continued use of the International Classification of Diseases, 9th Revision, Clinical Modification, Volumes 1 and 2 (diagnoses), and 3 (procedures) (ICD-9-CM), including the Official ICD-9-CM Guidelines for Coding and Reporting, through September 30, 2015.

PQRS Group Practice Reporting Option

The Centers for Medicare and Medicaid Services has announced that eligible professionals are now able to register for the group practice reporting option.

Registration for the 2014 PQRS program will be open until Sept. 30.

Group practices participating in GPRO that satisfactorily report data are eligible for a 0.5 percent incentive payment and will avoid the -2 percent 2016 PQRS payment penalty.

To earn the incentive payment and avoid the penalty, group practices may register via:

  • A qualified PQRS registry
  • Directly from EHR using certified EHR technology
  • CEHRT via data submission vendor

Groups with 25 or more eligible professionals may also participate via:

  • Web interface (reporting CG CAHPs also required for groups of 100 or more)
  • CG CAHPS via CMS-certified survey vendor

Fruits Vegetables and Mortality

A recent study in the British Medical Journal was published to examine and quantify the potential dose-response relation between fruit and vegetable consumption and risk of all cause, cardiovascular, and cancer mortality.

Prospective cohort studies were selected that reported risk estimates for all cause, cardiovascular, and cancer mortality by levels of fruit and vegetable consumption.

Results of this analysis found that there was a threshold around five servings of fruit and vegetables a day, after which the risk of all-cause mortality did not reduce further. A significant inverse association was observed for cardiovascular mortality, while higher consumption of fruit and vegetables was not appreciably associated with risk of cancer mortality.

The study concluded that this meta-analysis provides further evidence that a higher consumption of fruit and vegetables is associated with a lower risk of all-cause mortality, particularly cardiovascular mortality.

Movement Speed and Cervical Range of Motion

Cervical range of motion (RoM) has been the subject of many studies. However, only very few of these studies have considered the influence of movement execution speed on the cervical kinematics. The aim of this study is to evaluate the influence of movement speed on cervical RoM.

A recent study in the European Spine Journal looked at the primary spinal movements – flexion–extension, lateral bending, and axial rotation. Coupled movements were also studied. Paired Student’s tests were performed to compare the two modes of movement.

The results showed that cervical RoM differed significantly between movement speeds. Amplitudes were higher for each movement when movements were performed as quickly as possible. The range of movements carried out the best possible reached only 95 % of those during movements carried out as fast as possible. Concerning coupled movements, an increase in rotational movements coupled to lateral flexion during fast movements was observed.

The study concluded that range of motion reported in the literature corresponds to movement carried out in a mode resembling the best possible of our study. Movements made as quickly as possible can display larger motion ranges.
