Preparing for ICD-10

With the October 1, 2015 deadline for ICD-10 implementation rapidly approaching, here are some tips and steps to prepare for this ICD-10 transition.

  • Identify and plan to update all your systems to ICD-10 by making sure you understand your current systems and processes currently in use by ICD-9.
  • Communicate with your payers, clearinghouses, etc. to determine their system readiness and ask questions about what they’re doing to prepare for the transition.
  • Ask your payers if there are any contractual changes regarding coding specificity that could affect how you process claims.
  • Document any potential workflow changes.
  • Determine the training needs of your staff and aim to plan accordingly. Your staff will require different specific pieces of education around ICD-10 coding.
  • Plan for added expenditures in time and resources as you work to prepare your practice and your staff for the transition period.
  • Test, Test, Test. Before the deadline, make sure to test your transaction submissions with your vendors, payers and clearinghouses to make sure everything is going smoothly.

Craniosacral Therapy and Chronic Neck Pain

With growing evidence for Craniosacral Therapy (CST) effectiveness for pain management, the question about CST efficacy remained unclear.

A recently published study in The Clinical Journal of Pain aimed at investigating CST in comparison to sham treatment in chronic non-specific neck pain patients.

54 blinded patients were randomized to either 8 weekly units of CST or light touch sham treatment. Outcomes were assessed before and after treatment (week 8) and a further 3 months later (week 20).

The primary outcome was pain intensity on a visual analogue scale; secondary outcomes included pain on movement, pressure pain sensitivity, functional disability, health-related quality of life, well-being, anxiety, depression, stress perception, pain acceptance, body awareness, patients’ global impression of improvement and safety.

The study concluded that Craniosacral Therapy was both specifically effective and safe in reducing neck pain intensity and may improve functional disability and quality of life up to 3 months post intervention.

Neural Responses to Spinal Manipulation

A study was published in the journal Manual Therapy to determine how contact site for high velocity, low amplitude spinal manipulation (HVLA-SM) affects sensory input to the central nervous system.

Systematic investigations are needed identifying how variability in the biomechanical characteristics of spinal manipulation affects physiological responses. Such knowledge may inform future clinical practice and research study design.

high velocity, low amplitude spinal manipulation was applied to 4 specific anatomic locations using a no- high velocity, low amplitude spinal manipulation control at each location randomized in an 8×8 Latin square design in an animal model.

The study concluded that this animal study showed that contact site for an high velocity, low amplitude spinal manipulation can have a significant effect on the magnitude of sensory input arising from muscle spindles in the back.

Spinal Discs and Enzyme Treatment

Condoliase, an enzyme that degrades glycosaminoglycans, was well-tolerated by patients who received it as a treatment for lumbar disc herniations, according to a speaker here.

“Condoliase significantly improved symptoms and signs, physical function and quality of life scores in patients with lumbar disc herniations,” according to a presentation at the EuroSpine Annual Meeting.

The most common adverse event in the study was back pain, which was more evident in the condoliase group (36.6%) than the placebo group (33.3%), but most events were rated as mild or moderate.

Aetna Updates Reimbursement Policy for Code 97140

Dynamic Chiropractic recently published an article about Aetna’s revision of reimbursement for code 97140.

In a development the Association of New Jersey Chiropractors is calling “a resounding victory for chiropractors nationwide,” Aetna Insurance Company has updated its national reimbursement policy regarding 97140 (manual therapy), reaching an agreement two years after the association filed a declaratory judgment suit in federal court against the insurer.

Gone is the prepayment review policy of extraspinal diagnoses, which bundled 97140 into a chiropractic manipulative adjustment if performed on the same day and required documentation that the manual therapy was provided to a separate and distinct region from the CMT.

The updated policy also makes allowances for situations in which intraspinal claims qualify as separate, payable services.

“There are two major distinctions with Aetna’s post-settlement processing of claims for manual therapy (CPT 97140). First and foremost, Aetna previously required all 97140 claims to undergo prepayment review, which meant submission of notes with the claim, and waiting for an examiner to review the claims and then manually issue a pay or no-pay adjudication of the claim.

“Second, Aetna agreed to revise its policy language as it applies to prepayment review of intraspinal claims. Previously, if manual therapy was applied to any muscle in the same region as a chiropractic manipulative therapy, the claim would be automatically denied. The revised policy language recognizes that some soft tissues may span the same region as the CMT, but still qualify for separate reimbursement.

Thoracic Manipulation Mental State and Chronic LBP

The aim of a study published in the Journal of Physical Therapy Science, was to evaluate the changes in function and mental state after thoracic mobilization and manipulation in patients with chronic lower back pain (LBP).

Chronic lower back pain is defined as the LBP lasting over 3 months, and this pain could lead to social problems because treatment of LBP has many costs.

The Oswestry disability index (ODI) was used to measure the functional impairment of patients with LBP. A multiple spinal diagnosis was used to measure the range of motion (ROM) of vertebra segments.

The study concluded that application of mobilization or manipulation to thoracic lumbar vertebrae has a positive effect on function, mental state, and ROM in patients with lower back pain.
