Concussion Management Amongst Sports Physicians

The primary purpose of a study recently published in the Clinical Journal of Sports Medicine, was to examine concussion management practice patterns among sports medicine physicians in the United States.

Authors of the study distributed a questionnaire to physician members of the American Medical Society for Sports Medicine assessing the current practices for evaluating and managing concussions sustained during sports. Specifically, the authors asked respondents about their use of management guidelines, medications, balance assessments, neuropsychological tests, and return-to-play strategies.

The study concluded that most sports medicine physicians seem to use recently developed guidelines for concussion management, regularly use medications and neuropsychological testing in management strategies, and follow established return-to-play guidelines.

The Clinical Relevance Pearl of the study stated that Sports medicine physicians seem to have clinical expertise in the management of sport-related concussion.

Cervical Manipulation and Stroke

The American Chiropractic Association, the ACA has recently started to contact Doctors of Chiropractic about recent media stories related to Cervical Manipulation and risk of Stroke.

There is an ongoing interest in the association between cervical manipulation and the incidence of cervical arterial dissection (CAD) followed by stroke.

Historically, opinion rather than fact has often dominated discussions on this topic, even though the incidence of stroke associated with cervical manipulation is very rare and there is a lack of evidence to prove that cervical adjustments cause stroke.

The ACA is sensitive to the public’s concerns surrounding this complex issue, and is offering the following resources to help doctors of chiropractic disseminate accurate information to patients, health care colleagues and the media.

ACA encourages doctors of chiropractic to speak to patients about the benefits and risks associated with any treatment plan they may recommend, as well as benefits and risks associated with other treatments their patients may consider. Review ACA’s policy on Informed Consent for guidance on fostering shared decision making with patients.

The ACA recommends that all Doctors be prepared to speak with patients about chiropractic safety. To assist with these discussions, the facts sheets and infographic below may be downloaded, printed and distributed to patients.

PNF and Balance and Strength

A study published in the Journal of Bodywork and Movement Therapies described the effects of a proprioceptive neuromuscular facilitation (PNF) program on balance, strength, and mobility in an older adult with chronic stroke.

The patient was male, aged 69 years, with right hemiplegia for 17 years, and had diminished balance, balance confidence, lower extremity (LE) strength, and gait velocity. He received one hour of PNF-based therapy thrice a week for six weeks.

The study concluded that PNF was effective in enhancing balance, strength, and mobility in an older adult with chronic stroke and may mitigate falls risk in this population.

More research is needed to determine its impact in a larger sample of older people with chronic stroke.

Commentary on Cost Effectiveness of Manual Therapies

A commentary published in the Journal of Bodywork and Movement Therapies discussed the Cost Effectiveness of Manual Therapies for Neck and Low Back Pain.

Neck and low back pain (NLBP) are global health problems, which diminish quality of life and consume vast economic resources. Cost effectiveness in healthcare is the minimal amount spent to obtain acceptable outcomes. Studies on manual therapies often fail to identify which manual therapy intervention or combinations with other interventions is the most cost effective.

The purpose of this commentary is to sample the dialogue within the literature on the cost effectiveness of evidence-based manual therapies with a particular focus on the neck and low back regions.

The lack of homogeneity, in the available literature, makes difficult any valid comparison among the various cost effectiveness studies.

Potential outcome bias in each study is dependent upon the lens through which it is evaluated. If evaluated from a societal perspective, the conclusion slants toward “adequate” interventions in an effort to decrease costs rather than toward the most efficacious interventions with the best outcomes.

When cost data are assessed according to a healthcare (or individual) perspective, greater value is placed on quality of life, the patient’s beliefs, and the “willingness to pay.”
