Kinematic analysis for shoulder and pelvis coordination

The purpose of a study published in the journal Gait Posture, was to compare the kinematics of the shoulder and pelvis based on range of motion (ROM), angular velocity, and relative phase (RP) values during trunk axial rotation.

All participants were asked to perform axial trunk rotation activities at a self-selected speed to the end of maximum range in a standing position. The outcome measures included ROM, angular velocity, and relative phase on the shoulder and pelvis in the transverse plane and were analyzed based on the demographic characteristics between groups.

The results of this study indicated that there was a difference in pelvic rotation in the transverse plane between groups during axial trunk rotation.

It would be important to coordinate postural stability between the shoulder and pelvic girdles during ambulation; however, the pattern of trunk movement decreased with age due to possible pelvic stiffness in subjects with recurrent LBP.

Therefore, improved pelvic flexibility for coordinated trunk movement patterns would help subjects with recurrent LBP.

US Senate Passes Bill to Expand Chiropractic Services at Major VA Medical Centers

The U.S. Senate has approved legislation that will improve the delivery of benefits to America’s veterans, bringing them one step closer to gaining further access to the essential services provided by doctors of chiropractic (DCs) at major Department of Veterans Affairs (VA) medical centers. S. 1203, the 21st Century Veterans Benefits Delivery Act, passed under unanimous consent and has been referred to the U.S. House of Representatives, where it awaits action.

Section 102 of the Act, titled “Expansion of Provision of Chiropractic Care and Services to Veterans,” calls for the chiropractic benefit to be carried out at a minimum of two additional medical centers or clinics per Veterans Integrated Service Network (VISN) no later than two years after enactment of the bill, with the program being implemented at no less than 50 percent of all medical centers in each VISN within three years of enactment. There are nearly 160 VA treatment facilities nationwide. Currently, DCs serve at 66 treatment facilities across the country.

Especially important to note in the bill is the further integration of chiropractic services available to veterans, which was broadened to include services provided by DCs under the Preventive Health Services and Medical categories in addition to the existing coverage area of Rehabilitative Services, placing DCs in service categories previously closed to them.

NY Closure of Health Co-Op

The sudden collapse of the largest nonprofit insurance cooperative created by President Barack Obama’s health care law is causing headaches in New York, especially for medical providers owed millions of dollars for treating the failed plan’s patients.

More than 200,000 people insured through Health Republic Insurance of New York had until late November to sign up with another company if they want to maintain coverage in December.

State regulators ordered the insurer to shut down at the end of the month because of severe financial problems. They are also investigating what they say were inaccurate financial filings by the company.

The closure — part of a wave of failures of the new co-ops nationwide — has been a big hassle for Health Republic policyholders, who have had to shop around quickly for alternative coverage.

The situation may be worse, though, for doctors, hospitals and other clinicians. They are legally obligated to continue treating Health Republic patients through the end of the month but have been given no assurances they will ever be paid for that care.

MagnaCare, the company that had provided Health Republic with its large network of medical providers, tried to stem losses by ending its agreement with the insurer on Oct. 19, but the state intervened Nov. 5 and ordered the network to keep seeing patients through the end of the month, saying a shutdown would be too disruptive.

Officials in New York’s Department of Financial Services and the state’s health plan marketplace have said that for now they are focusing their energy on making sure Health Republic’s policyholders switch to another plan without a disruption in their care.

ACA President Updates on National Medicare Campaign

Recently, ACA President Dr. Anthony Hamm sent out the following letter to update the profession on the ACA’s National Medicare Campaign.  Following is a partial recap of this letter.

“Last month, ACA officially launched a national grassroots campaign designed to achieve full physician status for doctors of chiropractic in Medicare — an effort that seeks to correct the long-standing arbitrary and discriminatory limitation on our ability to be reimbursed for all necessary services that fall within individual state scopes of practice.

The launch of this campaign, the National Medicare Equality Petition, comes after many years of intensive behind-the-scenes efforts that sought to convince the Centers for Medicare and Medicaid Services (CMS) to issue regulatory changes that would address many of our concerns and those of Medicare beneficiaries.

After many years of work by both staff and volunteers, significant resources allocated to this effort and detailed supportive information repeatedly presented to CMS — the non-responsiveness of this agency clearly reflects an unreasonable bias against our profession.  While we will continue to dialog with CMS and press our concerns regarding a number of issues of vital importance to the profession, we are now prepared to take our concerns directly to Congress. We are determined to do so in in an ongoing and highly robust fashion that will ultimately replace an archaic law.

The success of our campaign will depend on your participation – and your ability to reach out to your patients, supporters, and colleagues to marshal the widespread support needed to sustain the intense grassroots lobbying that will pressure Congress into taking action.

Manipulation of the Feet

The purpose of a retrospective case series published in the Journal of Chiropractic Medicine, was to describe treatment outcomes for patients with plantar digital neuralgia (PDN) (Morton’s neuroma) who were treated using foot manipulation.

Charts were reviewed retrospectively for patients with a diagnosis of PDN and who received a minimum of 6 treatments consisting of manipulation alone. Visual analogue pain scales (VAS) and pressure threshold meter readings (PTM) were extracted as outcome measures.

The study concluded that the patients with plantar digital neuralgia who were included in this case series improved with conservative care that included only foot manipulation.

Medicare Local Coverage Determination

ChiroCode recently published the following concerning the Medicare Local Coverage Determination.

The Medicare LCD (Local Coverage Determination) is an excellent and necessary tool for appropriate Medicare coding. Medicare LCDs are also excellent tools for training as they contain a comprehensive list of ICD-10 codes that are anticipated to be commonly used among payers for chiropractic services.

Each state or region has established the Medicare LCD that is specific to that area. To avoid Medicare billing errors, providers should obtain their local LCD now and familiarize with those ICD-10 codes that are approved by Medicare.

Every Medicare Contractor has published the “Future” LCD for ICD-10 which went into effect on 10/1/2015. To obtain your Medicare LCD, you may simply search the Medicare Coverage Database or visit the website to your local Medicare Contractor and follow the tab labeled ‘LCDs’.

Providers are responsible for following current coding guidelines and should verify this information as the ICD-10 implementation date has occurred.
